"For visa applications, your health insurance must have no co-payments and no waiting periods. Standard Spanish policies often don't meet these requirements – you need a visa-specific policy from the start."

"Many expats under-insure on dental and optical. Adding these to your policy costs €10-20/month but saves hundreds when you need a filling or new glasses. It's one of the most common regrets I hear."

Health insurance FAQs for expats
Do I need private health insurance to get a Spanish visa?
Yes, for most visa types (non-lucrative, Golden Visa, student visa), you need comprehensive private health insurance that covers you in Spain with no co-payments and no waiting periods. The policy must be valid for the entire visa period. We work with premium insurers to provide visa-compliant policies with all required documentation.
What's the difference between Sanitas, Asisa, Adeslas & DKV?
All are top-tier insurers with different strengths. Sanitas: largest English-speaking network, strong in Madrid/Barcelona. Asisa: best value for money, extensive nationwide coverage. Adeslas: largest hospital network (part of Mapfre). DKV: premium service, higher prices, German expat favorite. We compare all options and recommend the best insurer for your specific location and needs.
What are pre-existing conditions and waiting periods?
Pre-existing conditions are health issues you had before getting insurance. Most policies have a 6-month to 2-year waiting period before they'll cover treatment for pre-existing conditions. Some insurers allow you to pay extra to reduce or waive waiting periods. We'll explain each insurer's policy clearly.
Can I use public healthcare if I have private insurance?
If you're registered with Spanish social security (as an employee or autónomo), you can still use public healthcare even with private insurance. Many expats keep both – public for emergencies and serious conditions, private for faster access to specialists and elective procedures.
How much does health insurance cost in Spain?
Costs vary by age, coverage level, and whether co-payments are included. Adults under 50: €45-140/month. Ages 50-65: €80-200/month. Over 65: €150-350+/month. Family plans offer discounts. We compare quotes from multiple insurers to find you the best price.
Can retirees and seniors get health insurance?
Yes, but options become more limited and expensive after age 65-70. Some insurers don't accept new customers over 70. Pre-existing conditions may be excluded or have long waiting periods. The earlier you apply, the better your options. We work with insurers that specialize in senior coverage.
What if I'm already insured in my home country?
For visa purposes, you need a policy specifically valid in Spain issued by an insurer authorized to operate in Spain. EHIC/GHIC cards (for EU/UK) don't count for visa applications. Travel insurance isn't sufficient either. You need a proper Spanish health insurance policy.
Do I need to speak Spanish to use private healthcare?
Not necessarily. Many private hospitals in expat areas have English-speaking staff and doctors. Some insurers offer translation services. We can help you find English-speaking doctors within your policy's network.
Can I add dental and optical cover?
Yes, most insurers offer dental and optical cover as optional add-ons or as part of premium plans. Basic plans typically exclude dental beyond emergency care. Dental cover usually includes check-ups, cleanings, and discounts on treatments. Optical cover may include eye tests and subsidies toward glasses.
What happens if I need to make a claim or see a doctor?
With private insurance in Spain, you typically don't 'claim' – you just visit approved doctors/hospitals who bill the insurer directly. Some plans have small co-payments (€5-20 per visit). We'll help you understand how to use your policy and can recommend English-speaking doctors.
Can I cancel my policy if I leave Spain?
Yes, policies can usually be canceled with 1-2 months' notice. You may receive a partial refund if you cancel mid-year. Some policies have minimum contract periods (e.g., 1 year). Notify your insurer promptly if you're leaving Spain permanently.
Is maternity covered by private health insurance?
Yes, but with waiting periods. Most policies require you to be insured for 8-12 months before maternity cover begins. If you're planning a pregnancy, factor this into your timing. Enhanced maternity cover with shorter waiting periods is available for higher premiums.
